Summary:
The East Texas Aging and Disability Resource Center will provide streamlined access to a wide range of aging and/or long-term services for persons who have intellectual disabilities or related conditions, persons with long-term needs which affect their ability to function in daily life and to persons who are elderly (60 years or older) and their caregivers.
It will serve the citizens of Gregg, Harrison, Marion, Panola, Rusk and Upshur Counties. The combined total population of these counties is approximately 298,909.
Community Healthcore is a multi-service agency with over 30 years of experience. The ADRC will be housed within in conjunction with its Mental Retardation Authority Services but is supervised independently by the Project Director. Although it is the sponsoring agency it is collaborating with other community partners including Long-term Services Region 4 & 5, The Area Agency on Aging of East Texas, East Texas Center for Independent Living, INFOline of Gregg County, Workforce Solutions East Texas and East Texas Border Health who are participating in the planning stage.
Community Healthcore will be providing the initial staffing but efforts are included in the plan to transition the system navigation function to a broader base of partners. Access points include a call center, internet web page, community links, and drop-in visit to East Texas ADRC. The total project budget is $121,000.
The East Texas Aging and Disability Resource Center will empower individuals, families and caregiver to make informed choices; streamline access to a wide range of long-term services; provide a resource for professionals in health and long-term services; and, serve as a visible and trusted resource and entry point for aging and long-term services and supports. |
